Postoperative Benefits of Cilostazol on Patients Undergoing Intervention for Chronic Limb Ischemia

Although prior studies have shown the short-term benefits of cilostazol on claudicant patients who underwent iliac or femoropopliteal revascularization procedures, literature with chronic limb ischemia (CLI) is limited. Recent molecular analysis suggests that possible cardiovascular are mediated through a nitric oxide pathway. As such, we hypothesized use in undergoing intervention for CLI would reduced major adverse events, reinterventions, and improved long-term survival. Vascular Quality Initiative (5/2012 to 10/2021) aortoiliac/femoropopliteal revascularizations were included. Patients stratified either no cilostazol, all heart failure excluded known contraindications. Incomplete covariate information was imputed. The groups’ clinical characteristics balanced using Inverse probability weighting. primary endpoint all-cause mortality. Secondary endpoints included perioperative events (stenosis/occlusion, thrombosis, amputation, dissection), 1-year (occlusion, revascularization). There 1824 26,029 final (Fig). group had lower incidence during admission (P < .001: 3.5% vs 4.8%) adjusted Kaplan-Meier showed significant 6-year survival difference 85% 79% cilostazol). In regression model, taking cilostazole preoperatively demonstrated odds hospitalization (odds ratio, 0.7; 95% confidence interval, 0.6-0.9; P .05) risk mortality (hazard .05 at 6 years).
